Working with Fraser Health Authority
The White Rock Hospice Society is partnered with the Fraser Health Authority's White Rock/South Surrey Hospice Palliative Care Program, which provides a team approach to coordination and provision of care for patients and families registered in the program.

A centralized record is utilized to assure continuity of care from the time of acceptance into the program until post bereavement services are completed.

Individuals with a life threatening illness may be referred to the program by professional caregivers or through self-referral at any time throughout the course of their illness.

Bereaved individuals are accepted into the program for grief counselling and support regardless of whether or not their loved one was registered with the program prior to death.
